What to Expect When You Visit Us
When you first come in for your examination, please bring any X-rays, CT scans, MRI scans or other studies that relate to your condition. If you have any hospital or physician records, please feel free to bring those as well. You will also need to bring your photo identification and insurance card. If you are suffering from an injury related to a worker’s compensation claim, please have all of your specific information available, such as the name of your employer, the claim number, contact person and phone number.
Every patient’s case is different, so it is hard to say how long you can expect to be in our office during your appointment. Your doctor should be able to give you a good idea of how long your visit will take. Please arrive 15 minutes early so that you will have plenty of time to fill out the necessary paperwork.
